When Should you go to the Emergency Room?
If you are experiencing any of the following symptoms, please call 911 or go to your nearest hospital emergency room:
- Cardiac chest pain
- Severe abdominal pain
- Severe difficulty breathing or shortness of breath
- Sudden dizziness, weakness or loss of coordination or balance
- Sudden blurred vision or loss of vision
- Numbness or weakness in your face, arm, or leg
- Sudden, severe headache
- Coughing up or vomiting blood
- Severe burns
- Seizures
The Appointment Procedure
In order to facilitate the appointment procedure, prior to scheduling an examination by telephone, you should have the following information:
- Name, address, telephone number, date of birth of the patient being scheduled.
- Insurance card, with the name of the insurance company, and the Social Security number of the member/employee covered by the insurance.
- Primary insurance information of dependent/spouse if covered by another insurance company.
- Secondary insurance information, if applicable.
