When Should you go to the Emergency Room?

If you are experiencing any of the following symptoms, please call 911 or go to your nearest hospital emergency room:

  • Cardiac chest pain
  • Severe abdominal pain
  • Severe difficulty breathing or shortness of breath
  • Sudden dizziness, weakness or loss of coordination or balance
  • Sudden blurred vision or loss of vision
  • Numbness or weakness in your face, arm, or leg
  • Sudden, severe headache
  • Coughing up or vomiting blood
  • Severe burns
  • Seizures

The Appointment Procedure

In order to facilitate the appointment procedure, prior to scheduling an examination by telephone, you should have the following information:
  • Name, address, telephone number, date of birth of the patient being scheduled.
  • Insurance card, with the name of the insurance company, and the Social Security number of the member/employee covered by the insurance.
  • Primary insurance information of dependent/spouse if covered by another insurance company.
  • Secondary insurance information, if applicable.
