Appointments

We reserve the right to reschedule appointments if a patient arrives more than 10 minutes late. Always bring your photo ID and insurance card.

New Patient Appointment

When calling for an appointment, please be prepared to provide the patient’s name, address, phone number, birth date, type of insurance, reason for appointment, and referring physician, if applicable.

Or

Confirm with your insurance carrier by calling the number listed on the back of your card.

Our scheduler, will try her best to schedule your appointment with Dr. Levine that best suits your needs or condition as soon as possible.

Please fill out the required forms and bring them with you on the day of your appointment. Remember to bring your photo ID and insurance card(s).

You may pay for your appointment with cash, a major credit, bank debit card or personal check.

We will contact your physician for any pertinent medical records, test and lab results.

Depending on complexity, you can expect your initial examination to take a hour.

Please allow 15 minutes prior to your scheduled appointment to complete the registration process.


Follow-up Appointments

To keep your patient demographic information updated, you will need to inform our office front desk of any changes such as address, phone number, place of business, or insurance since your last visit.

We must be able to contact you.

Our front desk will assist you with the patient registration process.

At check in your patient information will be verified to confirm that it is up-to-date.

If there are any outstanding balances on your account you will be expected to pay in full unless prior arrangements have been made with our Billing Department. Once this registration process is completed, Dr. Levine will be notified of your arrival.
